        This edition of the Manual has been completely updated and extensively revised to reflect the changes in fetal, perinatal, and neonatal care that have occurred since the fifth edition. In the Manual, we describe our current and practical approaches to evaluation and management of conditions encountered in the fetus and the newborn. We recognize that many areas of controversy exist, that there is often more than one approach to a problem, and that our knowledge continues to grow. In addition, this edition is the first collaborative effort with authors from two major neonatology programs, reflecting the move of two of the editors, Eric Eichenwald and Ann Stark, from Harvard to Baylor College of Medicine in Houston, Texas, and their ongoing collaboration with their colleague and co-editor John Cloherty.
        The Children's Hospital Boston Neonatology Program at Harvard has grown to 56 attending neonatologists and 18 fellows who care for more than 28,000 newborns delivered annually at the Beth Israel Deaconess Medical Center (BIDMC), the Brigham and Women's Hospital (BWH) (formerly the Boston Lying-In Hospital and the Boston Hospital for Women), Beverly Hospital, Metro West Medical Center, Saint Elizabeth's Hospital, South Shore Hospital and Winchester Hospital. They also care for the 650 neonates transferred annually to the NICU at Children's Hospital Boston for management of complex medical and surgical problems. Fellows in the Harvard Neonatal-Perinatal Fellowship Program train in addition to Children's Hospital, at Beth Israel Deaconess Medical Center, Brigham and Women's Hospital and Massachusetts General Hospital.
        The Neonatology Program at the Baylor College of Medicine includes 33 attending neonatologists, 12 pediatrician hospitalists, and 16 fellows who care for approximately 19,000 newborns delivered each year at Saint Luke's Episcopal Hospital, The Methodist Hospital, Ben Taub General Hospital, Saint Luke's Community Medical Center at the Woodlands, Methodist Willowbrook Hospital, Twelve Oaks Medical Center, and East Houston Regional Medical Center. They also care for the approximately 2,200 infants admitted annually to the Newborn Center at Texas Children's Hospital for management of complex medical and surgical problems, including congenital heart disease. Fellows in the Baylor College of Medicine Neonatal-Perinatal Fellowship Program train at Texas Children's Hospital, Ben Taub General Hospital, and Saint Luke's Episcopal Hospital.
        Programs in both institutions, The Children's Hospital Boston Advanced Fetal Care Center (in collaboration with the obstetrical programs of Beth Israel Deaconess Medical Center and Brigham and Women's Hospital) and The Texas Children's Fetal Center (in collaboration with the Texas Children's Hospital/Baylor College of Medicine obstetric program at Saint Luke's Episcopal Hospital) provide evaluation, genetic and obstetric fetal imaging and invasive diagnostics, and in utero or peripartum therapy to mother and child. Using interventional techniques, some problems are treated in utero to prevent neonatal disease.
        Our commitment to values including clinical excellence, collaboration with colleagues, and support of families is evident throughout our book. The hospitals mentioned earlier are referral centers for pregnancies complicated by maternal illness, fetal abnormalities, and anticipated neonatal problems. Obstetricians and neonatologists work together to provide prenatal, perinatal, and neonatal care. This perinatal experience is reflected in the chapters on Fetal Assessment and Prenatal Diagnosis, Maternal Conditions that Affect the Fetus, and Genetic Issues Presenting in the Nursery.
        The neonatal intensive care units (NICUs) at Children's Hospital Boston and Texas Children's Hospital provide telephone consultation to pediatricians at local community hospitals and facilitate advanced care for patients cared for by neonatologists at referring NICUs. Both programs transport and care for critically ill newborns throughout New England and Texas and the surrounding states. Frequent problems in this outborn population include perinatal asphyxia, congenital heart disease, severe respiratory disorders, malformations, surgical emergencies, and metabolic disorders. The practical approaches we have developed to address these problems are described in the Manual.
        Close involvement with the families is valued in our NICUs. This is reflected in the chapters on Breast Feeding, Developmentally Supportive Care, Management of Neonatal Death and Bereavement Follow Up, and Decision Making and Ethical Dilemmas.

        Paediatric radiography, despite being acknowledged as an imaging specialism, does not have a strong presence in either undergraduate or postgraduate radiography education programmes, and the availability of current published literature aimed at general radiographers is extremely limited. Consequently, the aim of Paediatric Radiography is to provide a reference text for radiographers and student radiographers working within general imaging departments and highlights aspects of paediatric healthcare that may influence paediatric radiography practice.
        Importantly, when writing this text, we have not sought to provide a description of all paediatric imaging techniques or provide answers to all imaging dilemmas, because many of these will be dependent upon local expertise, radiographic equipment and availability of alternative imaging modalities. Instead we have attempted to raise important aspects of paediatric healthcare that should inform radiographic practice and hope that these will be discussed openly within imaging departments. As a consequence of the current shortage of paediatric radiography texts we have considered literature from other health professions, particularly nursing, and have attempted to adopt some of their good practice models. Therefore this text may also be useful for nurses, physiotherapists and junior doctors interested in the imaging of children and its role in current paediatric healthcare practice.
        The development of this book has enriched our understanding of paediatric healthcare and the role of diagnostic imaging within the discipline. Our hope is that this book will help enhance paediatric radiographic practice to ensure that children attending imaging departments will receive informed and appropriate paediatric care.

        Among the most important developments in clinical medicine in recent years has been the rapid advancement in imaging technologies. These technologies allow clinicians to gain better insight into the pathophysiological processes underlying their patients’ illnesses. The practice of gastroenterology, perhaps more than any other in medicine, is a visual one and has been enriched by such advances. Modalities such as endoscopy, double-contrast radiography, computed tomography, isotopic scintigraphy, ultrasonography, magnetic resonance imaging, and positron emission tomography have facilitated greatly the approach to diagnosing gastrointestinal disorders. The imaging of tissues using standard microscopy has been augmented with newer methodologies such as immunohistochemistry, in situ hybridization, and confocal microscopy, not to mention even more experimental techniques. While we have included descriptions of these advances in the Textbook of Gastroenterology, the old adage that “a picture is worth a thousand words” could not be more applicable to anything other than to the teaching of gastroenterology. With this in mind, we have endeavored to provide the fourth edition of the Atlas of Gastroenterology with additional graphic material that enhances the reader’s understanding of the written material in the Textbook. The fourth edition of the Atlas expands on the material presented in the third edition by the addition of fi gures to existing chapters and by the addition of chapters covering new subject matter. The written text in the Atlas provides only an abbreviated introduction to the graphic material, and the reader is referred to the Textbook for more detailed information.
        Although the Atlas is meant to be especially useful to the reader of the Textbook, the quality of many of the fi gures is unique and not to be found readily in existing publications. Thus, we hope that the Atlas will serve as a valuable educational resource for all readers, independent of their familiarity with the Textbook.
        We are delighted to welcome Tony Kalloo to the editorial team for the fourth edition of the Atlas of Gastroenterology. His hands-on experience in the most modern procedural techniques of gastroenterology, in addition to his broad understanding of the discipline, have added immensely to this edition.
        We are most pleased as well to have a new publisher, Wiley-Blackwell, for this edition. Their keen insight into the publishing industry and the way in which textbooks are utilized today has been the basis for some of the changes made to the Atlas. In addition, their knowledge of the international world of medicine will help us to distribute the contents of the Atlas to a global audience.
