        The visual representations of common and atypical clinical manifestations of infectious diseases provide diagnostic information not found in the print version of the Red Book. The juxtaposition of these visuals with a summary of the clinical features , epidemiology, diagnostic methods, and treatment information serve as a training tool and a quick reference. The Red Book Atlas is not intended to provide detailed treatment and management information but rather a big-picture approach that can be re fined by consulting reference texts or infectious diseases specialists.

        The Red Book Atlas could not have been conceived without the dedication and tireless efforts of Edgar Ledbetter , MD , who for many years photographed disease manifestations in children.
        Many of these photographs were used in testing physicians during American Board of Pediatrics examinations. Some diseases rarely are seen today because of improved preventive strategies, especially vaccines. While photographs can't replace hands-on experience , they have always helped me to increase the likelihood of a correct diagnosis.
        The study of pediatric infectious diseases has been a challenging and changing professional life that has brought me great joy. To gather information with my ears and eyes (the history and physical examination) and to put the many pieces together to arrive at a diagnosis is akin to investigating a mystery. On many occasions, just seeing the clue (a characteristic rash , an asymmetry, a swelling) will solve the medical puzzle , lead to recovery , almost nothing can replace. It is my hope that the readers of the Red Book Atlas might find a similar enthusiasm for the field.

        The primary aim of this book is to provide the clinician with a well-illustrated text that may be used firstly to assist the diagnosis of those conditions that fall into the specialty of oral medicine and secondly to provide a guide to initial treatment. A number of excellent atlastype texts or reference works that comprehensively cover oral diseases are available. However, these books may be of limited value in the clinical setting since the material is usually presented according to the underlying etiology, such as infection, neoplasia, or developmental anomaly, rather than on the patient’s symptomatic complaint of ulceration, erythema, white patch, or pain.         The field of hepatology, including both the science and practice, encompasses a burgeoning knowledge base that has expanded substantially since the first edition of Handbook of Liver Disease was published in 1998. For example, there have been remarkable advances in the management of chronic hepatitis C, most notably the use of combination therapy with peginterferon and ribavirin, and improvements in the management of the troublesome side effects of antiviral therapy. There has been a marked expansion of therapeutic options for the management of chronic hepatitis B and improved understanding of the molecular biology of this virus, including characterization of the various spontaneous and drug-induced mutations and the hepatitis B virus genotypes. The growing importance of nonalcoholic fatty liver disease is becoming increasingly apparent, including its role in many cases of cryptogenic cirrhosis. Liver transplantation remains the standard of care for end-stage liver disease, and living donor liver transplantation has evolved as a viable option since publication of the first edition. There have also been remarkable changes in the approach to the diagnosis of many liver diseases, thanks to newer tests based on molecular techniques and advances in imaging.
        In the second edition of Handbook of Liver Disease, we have been fortunate to retain many of the pre-eminent contributors from the first edition and to welcome two new senior contributors, Dr Juan Rodés and Dr John Summerfield, whose addition enhances the international flavor of this book. Our goal in publishing this book remains the same – to provide a concise, accurate and up-to-date source of information that can be easily accessed.
        For this reason, we continue to use an outline format of focused paragraphs, short sentences, phrases, and lists, accompanied by figures and tables. The chapters remain concise yet in depth, and summarize the current practice of hepatology for all the major disease entities. An attempt has been made to use a uniform style among all the chapters while allowing each author flexibility in how best to convey the information. The purpose again is not to provide just the bare essentials, but rather the whole story in an economic format so that the richness and texture of the science that underlies the current practice of hepatology is readily apparent.
        While written with practicing gastroenterologists or hepatologists in mind, this book will also prove useful to primary care physicians, trainees in gastroenterology and internal medicine, and medical students rotating on a gastroenterology or hepatology service.

        The electrocardiogram (ECG) is an ideal tool for the practice of emergency medicinedit is non-invasive, inexpensive, easy to use, and it yields a wealth of information. All emergency physicians interpret multiple ECGs every day–and at times the most critical decisions of any given day are based on ECG interpretation at the bedside, such as in the assessment of the patients with chest pain, dyspnea, or even shock. However, although the ‘‘high profile’’ disease statesdsuch as acute coronary syndromedclassically are linked with this indispensable tool, we use the ECG for much more.
        Although traditionally the ECG is thought of as a cardiologists tool, it is really the domain of any medical practitioner making real-time assessments of patientsdthe emergency physician, the internist, the family practitioner, the intensivist, to name a few. As such, we all must become very comfortable with the many facets and subtleties of ECG interpretation. We should be expert in the urgent and emergent interpretation of the ECG. It is our hope that this issue of the Emergency Medicine Clinics of North America will help the physician on the front lines of patient care understand the complex wealth of information delivered by this relatively simple test.
        In this issue, we examine the ECG in traditional and nontraditional realms. Diagnosis of dysrhythmia and acute coronary syndromes is an obvious focus of this text. Several articles take an in-depth look at other morphologic issues we are often confronted with on the ECG; namely intraventricular conduction delays, the manifestations of electronic cardiac pacemakers, and the subtleties of ST segment/T wave changes as they pertain to the many syndromes that cause them. The issue also includes several articles on electrocardiographic manifestations of noncoronary disease, both cardiac and systemic. The ECG is also examined in subpopulations important to the emergency medicine practitioner: the child and the poisoned patient. Finally, more atypical topics of ECG interpretation are included; we offer an article on the detection of electrode misconnection and artifact, and look toward the horizon with a consideration of newer techniques and technologies.
        While working on this issue of the Emergency Medicine Clinics of North America, we considered not only healthcare provider education, but the constraints of rendering patient care in the emergency setting. We would like to recognize all emergency health care providers for their dedicated work for individuals in need. This work is performed at times under extreme circumstances with minimal information and resource. And yet, the outcome is most often positive. We should indeed all be proud of our profession.

        As I write this Preface in 2006, the specialty of obstetrics and gynaecology worldwide is going through a crisis of recruitment. It seems to all the contributors to this book a strange and sad time as the fascination of obstetrics and gynaecology remains unchallenged. Since the 6th edition, many advances have occurred and these have led to improvements in healthcare throughout the world. The efforts with regard to maternal and fetal health and to the gynaecological care of women remains a triumph of modern medicine. We hope that the reader will enjoy being stimulated by the fascination and intellectual stimulation that comes from the study of obstetrics and gynaecology.
        I wish to thank all the contributors who have submitted chapters for the readers’pleasur e, to impart their knowledge and to hopefully see this translated into high quality clinical practice. I would like to thank all of the contributors whose contributions span the breadth of obstetrics and gynaecology and we hope that these individual chapters will stimulate the reader to a greater understanding and thereafter a healthy appetite for more knowledge.
        The obstetrics and gynaecology of the future will almost certainly be different from the practice that has occurred over the last 150 years and as the future beckons more specialized individuals, a basic knowledge of obstetrics and gynaecology will still underpin the training of young doctors for the future.         The eleventh edition of Basic and Clinical Pharmacology is a new book in two important ways. At the same time, the overall organization has been improved and the educational content of previous editions has been expanded.

        During the course of this text’s ten editions, societal evolutions have materially transformed the provider-patient exchange. Each patient and each provider’s perception of these alterations may vary and there are certainly differences from country to country and within different cultures; however, cumulatively these evolutions amount to a changing paradigm of provider-patient interaction. In the US the majority of these transformations are positive.
        The provider-patient interface must allow the freedom for discussion of all such issues without threatening the individual beliefs, ethics, or morality of any party involved in the dialogue. True openness of discussion (listening as well as hearing) is imperative to guard against the hazard of ambiguous expectations and relationship difficulties.         French’s ‘Index’ was first published in 1912. The aim of this volume remains unchanged from the original statement by Herbert French in the first paragraph of his original preface; it is an alphabetic index to help in the differential diagnosis of any condition which may be seen in hospital or general practice. Essentially it is a book for the clinician. With modern transport, regional disease barriers have broken down. Moreover, the time it takes to get anywhere in the world is considerably less than the incubation period of almost all the infectious diseases. So, tropical illnesses are no longer confined to the tropics and one country’s epidemic may appear anywhere else in the world in record time. This, together with the massive increase in iatrogenic diseases, makes the art and science of differential diagnosis more interesting than ever before – and vastly more complex too!
        The first two editions of this book were edited by Herbert French. Subsequent editors, in turn, were Arthur Douthwaite, his colleague at Guy’s Hospital, then Sir Adolphe Abrahams of Westminster Hospital, and then Frank Dudley Hart, also of Westminster. The thirteenth edition had as its editors Professor Ian Bouchier of Edinburgh, the late Peter Fleming of Westminster Hospital, and Harold Ellis. For the fourteenth edition and in this updated fifteenth edition, Harold Ellis has been responsible for all topics of a ‘surgical’ nature, with Mark Kinirons responsible for the sections on ‘medical’ subjects.
        As for the contributors, we have retained a number of old friends and recruited new ones, all chosen carefully for their specialist knowledge and teaching skills. We thank them for their splendid work, although we take full responsibility for the contents of this book.
        ‘French’ has now been completely revised – many sections are largely rewritten, new ones added, diagnostic methods updated, many old illustrations replaced and others inserted. The emphasis, however, remains the same – the importance of a careful history, detailed clinical examination and the judicious use of laboratory and imaging investigations in the elucidation of the correct diagnosis.
        We hope that this new edition of French’s Index will continue to serve the medical profession, both in the United Kingdom and overseas, as it has done now for almost a hundred years.

        Among the most important developments in clinical medicine in recent years has been the rapid advancement in imaging technologies. These technologies allow clinicians to gain better insight into the pathophysiological processes underlying their patients’ illnesses. The practice of gastroenterology, perhaps more than any other in medicine, is a visual one and has been enriched by such advances. Modalities such as endoscopy, double-contrast radiography, computed tomography, isotopic scintigraphy, ultrasonography, magnetic resonance imaging, and positron emission tomography have facilitated greatly the approach to diagnosing gastrointestinal disorders. The imaging of tissues using standard microscopy has been augmented with newer methodologies such as immunohistochemistry, in situ hybridization, and confocal microscopy, not to mention even more experimental techniques. While we have included descriptions of these advances in the Textbook of Gastroenterology, the old adage that “a picture is worth a thousand words” could not be more applicable to anything other than to the teaching of gastroenterology. With this in mind, we have endeavored to provide the fourth edition of the Atlas of Gastroenterology with additional graphic material that enhances the reader’s understanding of the written material in the Textbook. The fourth edition of the Atlas expands on the material presented in the third edition by the addition of fi gures to existing chapters and by the addition of chapters covering new subject matter. The written text in the Atlas provides only an abbreviated introduction to the graphic material, and the reader is referred to the Textbook for more detailed information.
        Although the Atlas is meant to be especially useful to the reader of the Textbook, the quality of many of the fi gures is unique and not to be found readily in existing publications. Thus, we hope that the Atlas will serve as a valuable educational resource for all readers, independent of their familiarity with the Textbook.
        We are delighted to welcome Tony Kalloo to the editorial team for the fourth edition of the Atlas of Gastroenterology. His hands-on experience in the most modern procedural techniques of gastroenterology, in addition to his broad understanding of the discipline, have added immensely to this edition.
        We are most pleased as well to have a new publisher, Wiley-Blackwell, for this edition. Their keen insight into the publishing industry and the way in which textbooks are utilized today has been the basis for some of the changes made to the Atlas. In addition, their knowledge of the international world of medicine will help us to distribute the contents of the Atlas to a global audience. The editors would like especially to thank Elisabeth Dodds at Wiley-Blackwell, whose commitment to excellence has contributed materially to the quality of the book, and Alison Brown and Oliver Walter, without whose assistance this fourth edition of the Atlas would not have been published.
        Our efforts were especially facilitated by the expert assistance of Lori Ennis and Barbara Boughen, who collaborated as a team, complementing editorial talents with interpersonal skills to maintain the high quality of the text and deliver the manuscripts in a timely fashion.         The material has been revised extensively but our goal has been the same – to present, in a concise manner, the most important clinical, diagnostic and therapeutic aspects of fungal disease. In each chapter the clinical manifestations and management sections have been updated, and where appropriate, new emerging pathogens have been included.
        New sections include the molecular diagnosis of fungal infections and mycological aspects of the indoor environment. Medical mycology lends itself to illustration and we have included additional illustrative material in this edition. Throughout we have attempted to illustrates salient features of infection and to give an idea of the range of causative organisms including some illustrative photographs of macroscopic and microscopic morphology.
        Recent references have been added and the list of online resources has been updated. We have made every effort to ensure that our drug and dosage recommendations are accurate and in agreement with current guidelines. It should be noted that the formulations and usages of the different drugs described do not necessarily have the specific approval of the regulatory authorities of all countries. Because dosage regimens can be modified in the light of new clinical research findings, readers are advised to check the manufacturers’ prescribing information to see whether changes have been made in the recommended dosages, or whether additional contraindications for use have been introduced.
