        The primary intent of this book is to serve as a rapid review of ACLS principles and serve as a study aid to improve performance on the ACLS written and practical examinations. Questions themselves often contain a “pearl” reinforced in association with the question/answer.
       
        Many questions have answers without explanations. This is done to enhance ease of reading and rate of learning. Explanations often occur in a later question/answer. It may happen that upon reading an answer the reader may think—“Hmm, why is that?” Or, “Are you sure?” If this happens to you, GO CHECK! Truly assimilating these disparate facts into a framework of knowledge absolutely requires further reading in the surrounding concepts.         This Handbook of Respiratory Medicine has been written largely by specialist registrars, for specialist registrars. Three of the four authors, Stephen Chapman, Grace Robinson, and Sophie West, are specialist registrars on the Oxford rotation and John Stradling is Professor of Respiratory Medicine in Oxford. It is in a format that the authors would like to have had when they started their specialist registrar training. However, we hope that any health worker or student with an interest in respiratory medicine will find this text a rapid and useful reference source.
        The layout of the book tries to fulfil the requirement to be able to look up a topic quickly when the clinical need arises, but also to provide a bit more insight into the more difficult areas. Therefore the chapters are of necessity different in style and reflect the authors' own views on how to best approach and understand an area.
        The handbook is divided into five sections: clinical presentations and approaches to symptoms and problems; the clinical conditions themselves; supportive information; procedures; and useful appendices (also on the inside covers), containing more technical and reference information.

    SUMMARY :
        Emergency surgical (ES)admissions in the UK are increasing. Between 1998 and 2006, there was an 18% increase in ES admissions resulting in more than half a million emergency general surgical admissions in the UK during the financial year 2005–2006. Many of these patients with relatively mi- nor conditions were generally well and had short hospital admissions; however, a significant proportion were acutely unwell and required the full spectrum of surgical and critical care interventions and had prolonged hospital admission. In 2005–2006, there were more than 33,000 appendicectomies, more than 15,000 cases of acute pancreatitis and more than 3000 cases of diverticular perforation, taking up more than 300,000 hospital bed days. There were also a group of ES patients who were acutely unwell with sepsis and who pro- gressed to severe sepsis and organ failure. In 2005–2006, 990 patients died of acute pancreatitis, 1671 patients died of ei- ther a perforated or bleeding duodenal ulcer and 1934 pa- tients died of complications of diverticular disease.
        Different models exist for the organisation of emergency surgery care both in the UK and internationally. Increas- ingly, US centres have integrated ES into the trauma service providing a single service for all acute surgical patients. In the UK, the Royal College of Surgeons of England (RCS) has long been advocating the separation of emergency and elec- tive surgery in order to improve training and the efficiency of both work streams. Its 2007 publication ‘Separating Emer- gency and Elective Surgical Care’ contained recommenda- tions about how services should be organised to maximise training of the future surgical workforce and, most impor- tantly, improve patient care. Furthermore, in June 2007, the Association of Surgeons of Great Britain and Ireland (AS- GBI) published a consensus document containing essential service standards. It also discussed separation of specialist services and some institutions now divide the general on call between upper and lower gastrointestinal (GI) surgeons.

        We have a passion for improving patient care. Our journey with The Atlas of Emergency Medicine began with an aggressive goal of producing the most comprehensive source of highquality emergency department images available. The emergency department is, perhaps, the most diverse melting pot of patient conditions in the hospital. Diagnostic accuracy and prognostic prediction often rely heavily on visual clues. Our efforts are directed toward maximizing this skill. We also strongly believe the visual experience is critical to education in medicine, and that great images are the next best tool besides actual bedside exposure. Images often teach faster and with greater impact than many pages of text or hours of lecture.
        We continue our pursuit of these goals with a substantially updated, expanded, and improved third edition of The Atlas of Emergency Medicine. Nearly all of our changes and additions come from reader suggestions and criticisms, all of which we receive with sincere gratitude. First, we have changed the format to reduce text and allow for more images. Hence the text is more concise, providing only essential information. Each chapter item is now organized into: “Clinical Summary,” which includes pertinent differential diagnosis where appropriate, followed by “Emergency Department Treatment and Disposition” and, finally, “Pearls.” We have endeavored to provide “Pearls” that are more relevant and represent tips for diagnosis or unique aspects of a condition that are difficult to find in a typical text. Second, after extensive review and critique, hundreds of new and replacement images have been added.
        Third, four new chapters grace the pages of this new edition: Tropical Medicine, Toxicology, Airway, and Electrocardiography. Our increased emphasis on worldwide delivery of healthcare and easier patient travel is represented with Tropical Medicine. Toxicology is one of our core skills and a welcome addition as a separate chapter. We have made a decision to expand beyond our main emphasis on pictorial presentations with the addition of the Airway and ECG chapters. We included these topics, beautifully displayed in an atlas format, as they represent critical areas of emergency medicine expertise and are extremely visual. We believe they significantly contribute to the Atlas' ability to provide important visual information in a single source. These new chapters also complement our greatly expanded and updated Emergency Department Ultrasound chapter.
