    SUMMARY :
        The acquisition of knowledge is essential to creating diagnostic hypotheses and achieving a solution to the patient's clinical problem. With that in mind, we have developed the 5th edition of Kochar's Clinical Medicine for Students to link theoretical information more closely to the practice of medicine. Although direct patient experience is an irreplaceable way to develop the practice of medicine and patient care, theoretical knowledge is still an essential component of the learning process. Formal knowledge, which can be acquired from a book, serves as the basis for the application of knowledge in the context of the clinical practice, allowing the student to continuously learn from their experiences.
        This edition of Kochar's has undergone major changes and a significant transformation to meet the current needs of students in the health professions, in particular the needs of medical students in their third and forth years of medical school.
        The conceptual framework behind the development of this book was based on the ADDIE instructional model: Assessment of the learners' needs, Development, Demonstration, Implementation, and Evaluation of each section of the book. This process occurred constantly throughout the manuscript process, to ensure the creation of a text that was centered on the actual needs of the student.
        The 5th edition has been substantially revised, reorganized, and updated. Because the duration of courses or clerkships in medical schools is restricted, we decided to include only a selected number of key topics, which were chosen from two sources: Clerkship Directors in Internal Medicine (CDIM) core medicine clerkship curriculum guide, created by a national consensus of clerkship directors and academic general internists from the Society of General Internal Medicine (http://www.im.org/CDIM), and from USMLEโ„ข Step II Clinical Knowledge (CK) content outline (www.usmle.org).
        We also recognized that it was important not only for students to learn the typical presentation and diagnosis of common diseases, but also to recognize patients presenting with a specific complaint or finding, for which the generation of diagnostic hypotheses and their evaluation is essential. Finally, it was important to introduce a brief ambulatory medicine section to correspond to the ambulatory component included in many medicine clerkships.
        Another innovation of this edition is the collaborative effort between general internists and subspecialists; every chapter was written and reviewed by both groups. We believe that this cooperation will ensure accuracy, readability, and a more appropriate depth of content for medical students. In particular, the depth and relevance of content and the language used throughout the book were specifically revised. Additionally, 3rd and 4th year medical students acting as reviewers were involved in the development of the book from the beginning. Their feedback was most helpful, and their comments and suggestions were incorporated into the editing process.
        The organization and presentation of the material is divided into three sections: the diagnostic and clinical approach to common presenting complaints with particular attention to elements of the differential diagnosis; disease and disorders frequently encountered in medicine, formally described in a logical and structured manner; and principles of ambulatory medicine. This organization of content yields a broad, yet still selective, number of topics and approaches in inpatient and outpatient settings. We have organized the material so that it will make sense to 3rd year medical students who need to gain an understanding and knowledge of medicine, and to 4th year medical students who want to further their knowledge for clinical problem solving as well as prepare for the USMLEโ„ข Step II Clinical Knowledge exam. We also intend this book to be a resource for nurse practitioners and physician assistants who would like to review in a rapid and concise manner a number of medicine topics.

    SUMMARY :
        Paediatric radiography, despite being acknowledged as an imaging specialism, does not have a strong presence in either undergraduate or postgraduate radiography education programmes, and the availability of current published literature aimed at general radiographers is extremely limited. Consequently, the aim of Paediatric Radiography is to provide a reference text for radiographers and student radiographers working within general imaging departments and highlights aspects of paediatric healthcare that may influence paediatric radiography practice.
        Importantly, when writing this text, we have not sought to provide a description of all paediatric imaging techniques or provide answers to all imaging dilemmas, because many of these will be dependent upon local expertise, radiographic equipment and availability of alternative imaging modalities. Instead we have attempted to raise important aspects of paediatric healthcare that should inform radiographic practice and hope that these will be discussed openly within imaging departments. As a consequence of the current shortage of paediatric radiography texts we have considered literature from other health professions, particularly nursing, and have attempted to adopt some of their good practice models. Therefore this text may also be useful for nurses, physiotherapists and junior doctors interested in the imaging of children and its role in current paediatric healthcare practice.
        The development of this book has enriched our understanding of paediatric healthcare and the role of diagnostic imaging within the discipline. Our hope is that this book will help enhance paediatric radiographic practice to ensure that children attending imaging departments will receive informed and appropriate paediatric care.

        This book is a concise review of the medically important aspects of microbiology and immunology. It covers both the basic and clinical aspects of bacteriology, virology, mycology, parasitology, and immunology. Its two major aims are (1) to assist those who are preparing for the USMLE (National Boards) and (2) to provide students who are currently taking medical microbiology courses with a brief and up-to-date source of information. My goal is to provide the reader with an accurate source of clinically relevant information at a level appropriate for those beginning their medical education.
        This new edition presents current, medically important information in the rapidly changing fields of microbiology and immunology. It contains many color micrographs of stained microorganisms as well as images of important laboratory tests. It also includes a chapter on ectoparasites, such as the mite that causes scabies, and current information on antimicrobial drugs and vaccines.
        These aims are achieved by utilizing several different formats, which should make the book useful to students with varying study objectives and learning styles:
            1. A narrative text for complete information.
            2. A separate section containing summaries of important microorganisms for rapid review of essentials.
            3. Sample questions in the USMLE (National Board) style, with answers.
            4. A USMLE (National Board) practice examination consisting of 80 microbiology and immunology questions. The questions are incorporated into a clinical case format and simulate the computer-based examination. Answers are provided.
            5. Clinical case vignettes to provide both clinical information and practice for the USMLE.
            6. A section titled "Pearls for the USMLE" describing important epidemiological information helpful in answering questions on the USMLE.
            7. Many images of clinically important lesions seen in patients with infectious diseases described in this book are available on the McGraw-Hill Online Learning Center's Web site (www.langetextbooks.com). The lesions are shown in a gallery.
        The following features are included to promote a successful learning experience for students using this book:
            1. The information is presented succinctly, with stress on making it clear, interesting, and up to date.
            2. There is strong emphasis in the text on the clinical application of microbiology and immunology to infectious diseases.
            3. In the clinical bacteriology and virology sections, the organisms are separated into major and minor pathogens. This allows the student to focus on the clinically most important microorganisms.
            4. Key information is summarized in useful review tables. Important concepts are illustrated by figures using color.
            5. Important facts called "Pearls" are listed at the end of each basic science chapter.
            6. The 654 USMLE (National Board) practice questions cover the important aspects of each of the subdisciplines on the USMLE: Bacteriology, Virology, Mycology, Parasitology, and Immunology. A separate section containing extended matching questions is included. In view of the emphasis placed on clinical relevance in the USMLE, another section provides questions set in a clinical case context.
            7. Brief summaries of medically important microorganisms are presented together in a separate section to facilitate rapid access to the information and to encourage comparison of one organism with another.
            8. Fifty clinical cases are presented as unknowns for the reader to analyze in a brief, problem-solving format. These cases illustrate the importance of basic science information in clinical diagnosis.
            9. Seventy color images depicting clinically important findings, such as Gram stains of bacteria, electron micrographs of viruses, and microscopic images of fungi, protozoa, and worms, are included in the text.
        After teaching both medical microbiology and clinical infectious disease for many years, I believe that students appreciate a book that presents the essential information in a readable, interesting, and varied format. I hope you find this book meets those criteria.

        This edition of the Manual has been completely updated and extensively revised to reflect the changes in fetal, perinatal, and neonatal care that have occurred since the fifth edition. In the Manual, we describe our current and practical approaches to evaluation and management of conditions encountered in the fetus and the newborn. We recognize that many areas of controversy exist, that there is often more than one approach to a problem, and that our knowledge continues to grow. In addition, this edition is the first collaborative effort with authors from two major neonatology programs, reflecting the move of two of the editors, Eric Eichenwald and Ann Stark, from Harvard to Baylor College of Medicine in Houston, Texas, and their ongoing collaboration with their colleague and co-editor John Cloherty.
        The Children's Hospital Boston Neonatology Program at Harvard has grown to 56 attending neonatologists and 18 fellows who care for more than 28,000 newborns delivered annually at the Beth Israel Deaconess Medical Center (BIDMC), the Brigham and Women's Hospital (BWH) (formerly the Boston Lying-In Hospital and the Boston Hospital for Women), Beverly Hospital, Metro West Medical Center, Saint Elizabeth's Hospital, South Shore Hospital and Winchester Hospital. They also care for the 650 neonates transferred annually to the NICU at Children's Hospital Boston for management of complex medical and surgical problems. Fellows in the Harvard Neonatal-Perinatal Fellowship Program train in addition to Children's Hospital, at Beth Israel Deaconess Medical Center, Brigham and Women's Hospital and Massachusetts General Hospital.
        The Neonatology Program at the Baylor College of Medicine includes 33 attending neonatologists, 12 pediatrician hospitalists, and 16 fellows who care for approximately 19,000 newborns delivered each year at Saint Luke's Episcopal Hospital, The Methodist Hospital, Ben Taub General Hospital, Saint Luke's Community Medical Center at the Woodlands, Methodist Willowbrook Hospital, Twelve Oaks Medical Center, and East Houston Regional Medical Center. They also care for the approximately 2,200 infants admitted annually to the Newborn Center at Texas Children's Hospital for management of complex medical and surgical problems, including congenital heart disease. Fellows in the Baylor College of Medicine Neonatal-Perinatal Fellowship Program train at Texas Children's Hospital, Ben Taub General Hospital, and Saint Luke's Episcopal Hospital.
        Programs in both institutions, The Children's Hospital Boston Advanced Fetal Care Center (in collaboration with the obstetrical programs of Beth Israel Deaconess Medical Center and Brigham and Women's Hospital) and The Texas Children's Fetal Center (in collaboration with the Texas Children's Hospital/Baylor College of Medicine obstetric program at Saint Luke's Episcopal Hospital) provide evaluation, genetic and obstetric fetal imaging and invasive diagnostics, and in utero or peripartum therapy to mother and child. Using interventional techniques, some problems are treated in utero to prevent neonatal disease.
        Our commitment to values including clinical excellence, collaboration with colleagues, and support of families is evident throughout our book. The hospitals mentioned earlier are referral centers for pregnancies complicated by maternal illness, fetal abnormalities, and anticipated neonatal problems. Obstetricians and neonatologists work together to provide prenatal, perinatal, and neonatal care. This perinatal experience is reflected in the chapters on Fetal Assessment and Prenatal Diagnosis, Maternal Conditions that Affect the Fetus, and Genetic Issues Presenting in the Nursery.
        The neonatal intensive care units (NICUs) at Children's Hospital Boston and Texas Children's Hospital provide telephone consultation to pediatricians at local community hospitals and facilitate advanced care for patients cared for by neonatologists at referring NICUs. Both programs transport and care for critically ill newborns throughout New England and Texas and the surrounding states. Frequent problems in this outborn population include perinatal asphyxia, congenital heart disease, severe respiratory disorders, malformations, surgical emergencies, and metabolic disorders. The practical approaches we have developed to address these problems are described in the Manual.
        Close involvement with the families is valued in our NICUs. This is reflected in the chapters on Breast Feeding, Developmentally Supportive Care, Management of Neonatal Death and Bereavement Follow Up, and Decision Making and Ethical Dilemmas.
